Nobody can answer the EQ-question as it depends on your PA/your monitor, your needs, your likings etc.
Main Out Level = approx. -15 db.

This. mixing guys will love you for the -15db, otherwise lots of mixing desks begin to clip when being fed a hotter signal.
No one can answer this for you, it depends on the room, speakers being used even temperature as these can all affect the sound. My band has its own PA so I don't have to do much tweaking at gigs as I know what the system should sound like. We played a venue the other week which was made for bands room sounded awesome the next note we played some posh wine bar which had a wooden floor, mirrors on the wall it was so trebly and sounded like everything was caked in reverb so I had to adjust the output EQ to cater for this terrible room

That's true !
It's a good starting point to set the Output-level to -15 db and then adjust your rig-volumes for rhythm or lead-volumes. This procedure prevents from clipping the output section by too high rig volumes, because the profiles are levelled pretty high regularly. (Just my experience)

Remember anyway that the Mater volume doesn't set any level, it just imparts an attenuation.
